Fill in the blanks of the appointment request form with your contact information and check the box stating we may contact you directly to confirm the appointment. In the Comments Box of the appointment form, please include the following:
- The first day of your last menstrual period
- If you have had a pregnancy test, and if you have, the result
- Your date of birth
